is the general athlete medical insurance form. It MUST
be signed by the parents if the student athlete is under
the parent's insurance. |

of these forms are NECESSARY for new student athletes
coming to BGSU Athletics. They MUST be completed before
participation in atheltic practice. |

form is NECESSARY for all returning student athletes
before participation in athletic practice. This should
be completed as specificly as possible including all
injuries from last season. |

is a necessary release form that needs to be signed
in order for us to release your medical information
if necessary. |
